At Stone Community Care, our professional support coordination team is dedicated to assisting participants with navigating their NDIS plans. We are committed to understanding each individual's unique needs and guiding them in selecting the most suitable support program tailored to these requirements. Our knowledgeable and approachable team is ready to address all your enquiries and provide clarity on any aspects of the process.
Step 1
Initiate a meeting with your NDIA Planner to discuss your needs for Coordination of Supports, ensuring it is appropriately included in your plan.
Step 2
Fill out and submit your referral form to Stone Community Care Support Coordination. This task can be undertaken by you, your support planner, an NDIA Local Area Coordinator (LAC), a family member, or another designated representative.
Step 3
A Support Coordinator from our NDIS team will reach out to you or your appointed representative within three working days of receiving your form. The purpose of this contact is to confirm our collaboration with you or to provide an explanation in case Stone Community Care Support Coordination is unable to proceed with your referral.
Step 4
In the event that Stone Community Care Support Coordination currently lacks the capacity to take on your referral, you or the individual who referred you will be informed within 24 hours.
Participants receive guidance in executing their plans and establishing connections with their support systems.
This process is designed to enhance your competencies in engaging with a variety of support networks, including informal, mainstream, and funded services. By doing so, it aims to bolster your ability to sustain these support relationships, address service delivery challenges effectively, and independently participate in managing your NDIS supports and plans.

This includes managing critical situations, fostering capacity and resilience within your support networks, and efficiently orchestrating services from various providers to assist in fulfilling your specific needs and achieving your objectives.

The uppermost level of support, embedded in a therapeutic framework, is necessitated by particular high-level risks inherent in an individual’s situation.
This approach concentrates on addressing severe risk factors and complex challenges, with the goal of reducing the complexity of the support environment. It also involves assisting you in connecting with appropriate supports and services.
