At Stone Community Care, we specialise in providing tailored assistance and support for daily personal activities, catering to individuals who require supervision in their everyday lives. Recognising that determining the level of assistance needed can be challenging, we align our services with the guidance provided by the NDIS plan to ensure optimal support. Our assistance is offered in two primary forms: as part of assisted accommodation and within the personal residences of our participants.
Our commitment is to empower participants to lead lives directed by their own choices. To achieve this, we meticulously design our care supports to cater to the individual needs of our participants, fostering an independent lifestyle. Our services encompass a wide range of daily activities, including assistance with meals, dressing, and sleep routines. Beyond these, we extend our support to include medical appointments, recreational activities, educational visits, and more, ensuring a comprehensive and responsive care experience.
